    Make an appointment today to meet with our Community Health Coordinators to receive assistance with:

    - Accessing basic needs such as food, transportation, clothing and referrals to state benefits
    - Connecting to Community Resources including housing, utility assistance, healthcare and employment services
    - Obtaining educational training and career readiness support.
    - Health Education, health literacy and prevention services.
